Scheduling

If you have a medical emergency, please call 911 or go directly to the nearest emergency department.

To schedule an appointment:

• Call us directly at 570-322-8448 during regular office hours.
• Use our Request an Appointment form below, and we will call you within one business day to schedule a visit.

Canceling

If you can no longer attend your appointment, please call 570-322-8448 at least 24 hours before your appointment to cancel or reschedule an appointment.

Mask Requirements

COVID-19

Mask requirements will be determined by COVID-19 community transmission levels. When community transmission is medium or high, you are required to wear a mask at all times in the office and regardless of vaccination status. If you do not have a mask, a disposable one will be provided upon arrival at the office. When community transmission is low, you are encouraged to wear a mask in the office. The community transmission level will be posted at the entrance of the offices.
